This is a list of active Medicaid fee-for-service (FFS), Managed Care Only and Ordering, Prescribing, Referring, Attending (OPRA) providers. Data elements include the provider’s Medicaid Provider ID, NPI, provider or facility name, Medicaid type, profession or service, provider specialty, service address, city, state, zip code, county, telephone number, latitude, longitude, enrollment begin date, next anticipated revalidation date, date the file was extracted from the data warehouse and a Medically Fragile Children and Adults Directory Indicator.

This dataset includes information on the number of tests of individuals for COVID-19 infection performed in New York State beginning March 1, 2020, when the first case of COVID-19 was identified in the state. The primary goal of publishing this dataset is to provide users timely information about local disease spread and reporting of positive cases. The data will be updated daily, reflecting tests completed by 12:00 am (midnight) the day of the update (i.e., all tests reported by the end of the day on the day before the update).

Note: On November 14, 2020, only 14 hours of laboratory data was collected and shared. A 2:00 pm cutoff time was implemented, allowing the NYSDOH to enhance data quality reviews. All other published laboratory data represented 24 hours of data collection.

As of April 4, 2022, the Department of Health and Human Services (HHS) no longer requires entities conducting COVID testing to report negative or indeterminate antigen test results. This may impact the number and interpretation of total test results reported to the state and also impacts calculation of test percent positivity. Because of this, as of April 5, 2022, test percent positivity is calculated using PCR tests only. Reporting of total new daily cases (positive results) will continue to include PCR and antigen tests.

Reporting of SARS-CoV2 laboratory testing results is mandated under Part 2 of the New York State Sanitary Code. Clinical laboratories, as defined in Public Health Law (PHL) § 571 electronically report test results to the New York State Department of Health (DOH) via the Electronic Clinical Laboratory Reporting System (ECLRS). The DOH Division of Epidemiology’s Bureau of Surveillance and Data System (BSDS) monitors ECLRS reporting and ensures that all positives and negatives are accurate. Starting September 30, 2020, this data also includes pooled/batch tests reported by institutions of higher education. This is also known as surveillance testing and not performed by a clinical laboratory.

Test counts reflect those reported to DOH each day. A person may have multiple specimens tested on one day, these would be counted one time, i.e., if two specimens are collected from an individual at the same time and then evaluated, the outcome of the evaluation of those two samples to diagnose the individual is counted as a single test of one person, even though the specimens may be tested separately. Conversely, if an individual is tested on more than one day, the data will show two tests of an individual, one for each date the person was tested. An individual will only be counted positive one time.

Test counts are assigned to a county based on this order of preference: 1) the patient’s address, 2) the ordering healthcare provider/campus address, or 3) the ordering facility/campus address.

This data includes the name and location of food service establishments and the violations that were found at the time of their last inspection. Although violation details are collected on inspection reports (i.e., the actual food item, quantity and temperature of food found out of temperature control) as well as corrective actions for critical violations, this data set is limited to the violation number and the corresponding general violation description. This dataset is for reporting purposes only. Any concerns about individual establishments should be referred to the corresponding Local Health Department. The Genealogical Research Death Index assists individuals with locating New York State (NYS) death records that fall within defined genealogy years, exclusive of New York City recorded death records. This Index contains information on decedents, date of death, gender, age at death, and NYS file number that will allow the public to search for individuals on a variety of genealogy criteria if on file for at least 50 years.

The contract laboratory provides the radon test results to the individual home owner and the Radon Program. All testing data is entered into our database. From this database, we are able to create radon prevalence maps, design special outreach activities and campaigns, and track the location in the home where the detector was placed.

This dataset contains the locations of Article 28, Article 36, Article 40, and Article 7 health care facilities and programs from the Health Facilities Information System (HFIS). Article 28 facilities are hospitals, nursing homes, diagnostic treatment centers and midwifery birth centers. Article 36 facilities are certified home health care agencies, licensed home care services agencies, and long term home health care programs. Article 40 facilities are hospices. Article 7 are licensed adult care facilities.

The dataset currently only contains the locations of hospitals and hospital extension clinics. The data for the remaining facility types will be added in the future.

The Statewide Planning and Research Cooperative System (SPARCS) Inpatient De-Identified dataset contains discharge level detail on patient characteristics, diagnoses, treatments, services, and charges. This data contains basic record level detail regarding the discharge; however, the data does not contain protected health information (PHI) under Health Insurance Portability and Accountability Act (HIPAA). The health information is not individually identifiable; all data elements considered identifiable have been redacted. For example, the direct identifiers regarding a date have the day and month portion of the date removed.

This dataset contains information submitted by New York State Article 28 Hospitals as part of the New York Statewide Planning and Research Cooperative (SPARCS) and Institutional Cost Report (ICR) data submissions. The file contains information on the volume of discharges, All Payer Refined Diagnosis Related Group (APR-DRG), the severity of illness level (SOI), medical or surgical classification the median charge, median cost, average charge and average cost per discharge.

The Professional Medical Conduct Board Actions data consist of all public actions taken against physicians, physician assistants, specialist assistants, and medical professional corporations pursuant to Section 230 of the New York State Public Health Law and Section 6530 of the New York State Education Law.

This dataset includes information at the reporting facility level on patients hospitalized, admitted, discharged and fatalities. It also includes information on staffed beds. Patient information collected as part of the HERDS Hospital Survey are lab-confirmed COVID-19 positive. Hospitalized means patients admitted as inpatients in either inpatient or observation beds and does not include patients that were treated and released from an Emergency Department. The title of this dataset was initially the Hospital Electronic Response Data System (HERDS) Hospital Survey: COVID-19 Hospitalizations and Beds. The dataset was changed to its current title on 11/4/2021.

The datasets contain number of Medicaid PQI hospitalizations (numerator), county Medicaid population (denominator), observed rate, expected number of hospitalizations and rate, and risk-adjusted rate for Agency for Healthcare Research and Quality Prevention Quality Indicators – Adult (AHRQ PQI) for Medicaid enrollees beginning in 2011.

The School Immunization Survey collects aggregate data from schools in New York State regarding the immunization status of all students attending school. All schools, excluding New York City public schools, have reported the immunization status of all students in grades kindergarten through 12.

This data includes the name and location of active food service establishments and the violations that were found at the time of the inspection. Inspections are a snapshot in time and are not always reflective of the day-to-day operations and overall condition of an establishment.

The Nursing Home Weekly Bed Census: Last Submission data is the most current nursing home data available. Available bed and occupancy information is self-reported and is not audited by the Department of Health.

The Department of Health requires nursing homes to complete electronic filing of each facility's licensed nursing home beds and availability by bed category on a weekly basis. This dataset is refreshed monthly.

The Statewide Planning and Research Cooperative System (SPARCS) is a comprehensive data reporting system which collects patient level detail on patient characteristics, diagnoses, treatments, services, and charges for every hospital discharge from an Article 28 facility; ambulatory surgery discharges from hospital-based ambulatory surgery centers and all other facilities providing ambulatory surgery services; and emergency department visits in New York State. This dataset is a summary of the SPARCS inpatient discharge data.

The New York State Nursing Home Quality Initiative (NHQI) is an annual evaluation and ranking of eligible Medicaid-certified nursing homes in New York State. Nursing homes are evaluated on their performance in three components: Quality, Compliance, and Efficiency. Nursing homes are awarded points for their performance in each measure and ranked into overall quintiles, the first quintile containing the best performing homes.

This dataset contains information on certifications for services and beds for Article 28, Article 36, Article 40, and Article 7 health care facilities, and programs from the Health Facilities Information System (HFIS).

The Student Weight Status Category Reporting System (SWSCR) collects weight status category data (underweight, healthy weight, overweight or obese, based on BMI-for-age percentile). The dataset includes separate estimates of the percent of students overweight, obese and overweight or obese for all reportable grades within the county and/or region and by grade groups (elementary and middle/high).

New York State Baby Names are aggregated and displayed by the year, county, or borough where the mother resided as stated on a New York State or New York City (NYC) birth certificate. The frequency of the baby name is listed if there are 5 or more of the same baby name in a county outside of NYC or 10 or more of the same baby name in a NYC borough.

This dataset, prepared by the Bureau of Biometrics and Health Statistics within the Department of Health, is intended to identify individual registration districts in New York State. These districts are used to identify residences and places of occurrence of events reported as part of the vital records registration system of the New York State Department of Health.

This dataset aggregates and displays the number of New York State Medicaid enrollees by eligibility year and month within each NYS Economic Region, health insurance plan information, and enrollee demographics.

As a way of monitoring managed care plan performance and improving the quality of care provided to New York State residents, the New York State Department of Health (NYSDOH) collects data for four types of managed care insurance (commercial HMO, commercial Preferred Provider Organization, Medicaid managed care, and Child Health Plus) on an annual basis.

All acute care hospitals are required to report certain hospital-acquired infections (HAIs) to the New York State Department of Health (NYSDOH). This includes central line-associated blood stream infections; surgical site infections following colon, hip replacement/revision, coronary artery bypass graft, and hysterectomy; Clostridium difficile infections; and carbapenem-resistant Enterobacteriaceae infections.

The dataset is representative of financial incentive payments disbursed to Eligible Professionals (EPs) and Eligible Hospitals (EHs) by the NY Medicaid Electronic Health Records (EHR) Incentive Program, a CMS Promoting Interoperability Program. Authorized by the American Recovery and Reinvestment Act (ARRA) of 2009, this program provides multi-year financial incentives to EPs and EHs who adopt, implement, or upgrade, and subsequently demonstrate meaningful use of Certified Electronic Health Record Technology (CEHRT).
